Appointments
You may request an appointment online through our APPOINTMENT REQUEST page. Please understand that appointments are not confirmed until you receive telephone confirmation from us.

Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient.

Payment Policy
We participate with most insurance companies, as well as Medicare and Medicaid. We will submit all primary and secondary claims for you. You will be held responsible for any deductibles and coinsurance as required by your insurance policy.

We ask that you pay any co-pays on your office visit at the time of the office visit. If you ever have any concerns about your bill, please let us know either by calling the office or discussing this with us at the time of your visit.

Our billing office is available to work with you to answer any questions or problems that arise with insurance coverage. We also accept Visa, MasterCard and we participate with Care Credit for your convenience.
